Lifeline Program Terms

By enrolling in the Lifeline Program with Centranet, subscribers certify that they have read, understand and agree to the following: 

Prove your eligibility

Lifeline is only available to subscribers who can prove their eligibility. Subscriber must show proof of participation in a qualifying program or proof of income when applying for Lifeline.

Recertify every year

Every year, USAC will check to confirm a subscriber still qualifies. If USAC cannot confirm qualification, subscribers will receive a letter in the mail and may also receive reminders by e-mail or pre-recorded messages by phone. Subscriber must recertify within 60 days to keep the Lifeline benefit.

Keep Centranet up-to-date

Subscribers who have a change of address, no longer qualify for Lifeline, or more than one person in the subscriber’s household gets Lifeline, must notify Centranet within 30 days.

One discount per household

  • Only one discount per household. A “household” is a group of people who live together and share income and expenses.
  • I understand that due to the nature of this program, the Lifeline Program will not be prorated for a partial month of service and may be less than the full benefit during the final month of the program when program funding is nearing depletion.

Use it or lose it

Subscribers must use their service at least once every 30 days. A 15-day notice will be sent to use the service or it will be turned off.

Be honest on your application and forms

It is against the law to lie on any Lifeline-related forms or questionnaires.
